This new partnership offers a seamless method of continuing care for those in fragile health so they can make a smooth transition back home after a stay in a hospital or in a skilled nursing facility. The new HomeCare Maryland extends the reach of LifeBridge Health’s comprehensive services to make it convenient to people who need continuing care.
