Neptune, NJ – November 12, 2014 – Six Meridian Health physicians have been recognized by the National Committee for Quality Assurance for the exceptional care they provide. The National Committee for Quality Assurance (NCQA) is a not-for-profit organization committed to improving health care by measuring a number of quality standards and performance measures.
The three primary NCQA performance areas include the Diabetes Recognition Program (DRP), Heart/Stroke Recognition Program (HSRP) and Patient Centered Medical Home (PCMH) recognition program.

These physicians worked in conjunction with their practice team and Meridian’s physician enterprise support team to implement new processes and provide the necessary data to NCQA to receive these recognitions.

The Diabetes Recognition Program and Heart Stroke Recognition Program provide clinicians with tools to support the delivery and recognition of consistent high quality care. These voluntary programs are designed to recognize clinicians who use evidence-based measures and provide excellent care to their patients with diabetes.
The NCQA Patient Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication, and patient involvement.
The Patient-Centered Medical Home 2011 program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP) and American Osteopathic Association (AOA) and others in extension of the Physician Practice Connections Program.
