Neptune, NJ – November 12, 2014 – Six Meridian Health physicians have been recognized by the National Committee for Quality Assurance for the exceptional care they provide. The National Committee for Quality Assurance (NCQA) is a not-for-profit organization committed to improving health care by measuring a number of quality standards and performance measures.

The three primary NCQA performance areas include the Diabetes Recognition Program (DRP), Heart/Stroke Recognition Program (HSRP) and Patient Centered Medical Home (PCMH) recognition program.

Six Meridian Health Doctors have been recognized for meeting all three of these high standards:• Robert Carracino, M.D., Shrewsbury Internal Medicine• Anthony DeNoia, M.D., Atlantic Highlands Internal Medicine• Nicola DiGuglielmo, M.D., Monmouth Family Medicine Group, Ocean Townhip• Scott Druckman, D.O., Jackson Family Medicine• John Gumina, M.D., Monmouth Family Medicine Group, Freehold and Howell locations• Robert Shavelson, M.D., Tinton Falls Internal Medicine

These physicians worked in conjunction with their practice team and Meridian’s physician enterprise support team to implement new processes and provide the necessary data to NCQA to receive these recognitions.

“Practices that achieve all three levels of recognition are considered to be Triple Crown Winners,” shares James Clarke, M.D., vice president, primary care division of Meridian Physician Partners. “This achievement is another example of Meridian Health’s commitment to advancing new and evidence-based models of care to provide the best health care experience for our patients.”

The Diabetes Recognition Program and Heart Stroke Recognition Program provide clinicians with tools to support the delivery and recognition of consistent high quality care. These voluntary programs are designed to recognize clinicians who use evidence-based measures and provide excellent care to their patients with diabetes.

The NCQA Patient Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication, and patient involvement.

The Patient-Centered Medical Home 2011 program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP) and American Osteopathic Association (AOA) and others in extension of the Physician Practice Connections Program.

“The NCQA Patient-Centered Medical Home is a model of 21st century primary care that combines access, teamwork and technology to deliver quality care and improve health”, said NCQA president Margaret E. O’Kane. “NCQA Recognition shows that these physicians and practices have the tools, systems and resources to provide their patients with the right care at the right time.”
