Newswise — TOLEDO, Ohio, August 22, 2016 – The Root Cause Coalition, a nonprofit organization dedicated to addressing hunger and other social issues that impact health, will host a National Summit on the Social Determinants of Health Dec. 5 – 6 at The Drake Hotel in Chicago, Ill. The event will bring together thought leaders from across the nation to share dynamic data-driven, community-centered solutions to better address basic needs that, in turn, help prevent the exacerbation of chronic health conditions among the nation’s most vulnerable populations.

“There are clear and documented associations between social determinants and the poor health status of children, adults and seniors,” said Barbara Petee, Executive Director, The Root Cause Coalition. “Our aim is to harness the leadership power of health care and community organizations to create sustainable change in the areas of nutrition and food security, housing, education, transportation and isolation.”

Additionally, there will be breakout sessions focusing on best practices in addressing social determinants, led by representatives from leading organizations, including the Center for Health Law and Policy Innovation at Harvard Law School, Feeding America, Ascension Health, Blue Cross Blue Shield of Minnesota, Loma Linda University Health, OSF HealthCare, Humana, Advocate Health, St. Joseph Mercy Health System and UPMC Advantage.
