In a study published in the May 2022 issue of Diseases of the Colon & Rectum, investigators from the Medical University of Vienna, Austria, provide insight in the anatomical position of the mesh in relation to the sacrum during a laparoscopic procedure. They studied 18 fresh cadavers and performed laparoscopic sacral mesh fixation as surgeons would do during laparoscopic ventral mesh rectopexy.

Four major emerging categories for quality improvement in these patients include using prehabilitation, providing multidisciplinary care in partnership with geriatricians or practitioners with geriatrics expertise, adopting programs and techniques aimed at reducing stress during and after surgery, and assessing goals of care based on a consideration of realistic outcomes.

The recommendations were developed by the Pelvic Floor Disorders Consortium, a multidisciplinary organization representing the wide range of professionals involved in diagnosis and treatment of pelvic floor disorders. The new recommendations are now available on the DC&R website and appear in the journal's October issue. Liliana Bordeianou, MD, MPH, of Massachusetts General Hospital Pelvic Floor Disorders Center and Harvard Medical School was senior author of the consensus statement.

ASCRS Publishes New Guidelines for Colonic Volvulus and Acute Colonic Pseudo-Obstruction

Colonic volvulus, or twisting of the large intestine, is a potentially life-threatening cause of large-bowel obstruction. An updated set of evidence-based recommendations for management of colonic volvulus – as well as for a rare condition called acute colonic pseudo-obstruction (ACPO), which can mimic the symptoms of large bowel obstruction – will appear in the September 2021 issue Diseases of the Colon & Rectum (DC&R), the official journal of the American Society of Colon and Rectal Surgeons (ASCRS). The journal is published by Wolters Kluwer.

In the January issue of Diseases of the Colon & Rectum, Dr. Sahnan and colleagues from the Fistula Research Unit at St. Mark’s Hospital and Academic Institute, Harrow, United Kingdom report results of a large study evaluating an 11-year experience at their unit including nearly 2000 patients with perirectal abscess. One out of every 6 perirectal abscess patients went on to develop an anal fistula, most about 7 months after the abscess had occurred.

In the January issue of Diseases of the Colon & Rectum, surgeons from 3 large universities (Pennsylvania State, Temple and Columbia) examined outcomes of patients undergoing emergency colon surgery at their institutions. They wanted to find out whether there was a difference in outcomes when surgery was performed by a general surgeon, a surgeon specializing in emergency surgery, or a colon surgery specialist. In order to avoid biasing the study, patients operated on by different groups of surgeons were “matched” so that there was no difference in the severity of their illness or in the number of other health problems in patients between groups.
