Patients vaccinated at Hackensack Meridian Health can now use the free CLEAR app and digital vaccine card feature - replacing their fragile paper card with a verified digital version that can be used anywhere and anytime needed. 

CLEAR's digital vaccine card is a free, mobile experience on the CLEAR app, which securely connects a person's verified identity to their proof of vaccination, to reduce public health risk and get people back to what they love. CLEAR’s digital vaccine solutions are being used by more than 130 organizations across the country, including the New York Stock Exchange, the Public Theater, and Lincoln Center. CLEAR has also been named an approved platform for vaccine verification by New York City, San Francisco, Los Angeles, and Seattle.

In addition to CLEAR’s digital vaccine card, Hackensack Meridian Health patients have access to other digital tools including the HMH Well App and MyChart. The HMH Well App, available on iTunes and Google Play, gives patients the ability to manage their health care needs; get directions to the nearest facilities and view emergency department (ED) wait times; find a doctor by name, location and specialty; and access their MyChart account. Through the MyChart feature, patients can book an appointment, see their lab results, communicate with their doctor's office, request a prescription refill and track health information. Patients can access these three tools by following these steps:
