The American Academy of Home Care Medicine (AAHCM) applauds the Senate Finance Committee for passage of legislation, this week, that includes extension and expansion of the Independence at Home (IAH) demonstration through the CHRONIC Care Act of 2017. The IAH provisions of the bill extend the program for an additional two years past its September 2017 expiration, increase the cap on the total number of participating beneficiaries from 10,000 to 15,000, and improve the ability of IAH programs to best serve their patients.

The Academy is pleased to share that the Independence at Home (IAH) Demonstration saved Medicare approximately $33 million in Year 4, or about $384 per beneficiary per month (PBPM). This follows savings from Years 1-3 that, when combined with Year 4, amount to about $63 million in savings and high-quality care for Medicare. These results highlight the value of a home-based primary care (HBPC) model to serve complex, frail elders, with an emphasis on a high-touch and high-tech mobile team care.

The American Academy of Home Care Medicine (AAHCM) is thrilled to announce that The John A. Hartford Foundation (JAHF) has awarded AAHCM a $340,000 grant to create a Home-Based Primary Care National Practice Directory. The John A. Hartford Foundation has also made grants for a Workforce and Education initiative from the Home-Centered Care Institute (HCCI) and a national Learning Collaborative for Home-Based Primary Care and Palliative Care.

The Academy is pleased to share that the Independence at Home (IAH) Demonstration Year 5 practice expenditures were $33.5 million below Medicare spending targets. Over the 5 years of the demonstration, the IAHC practices have generated a total of about $100 million in savings and high-quality care for Medicare.

To help spread awareness of the power of home care medicine, AAHCM partnered with Information Matrix, producers of a series of award-winning educational videos to create a documentary entitled “The Power of Home Care Medicine.” The documentary is available as both a full length, six-minute version and an abridged, one-minute version.

In the Fall of 2019, The John A. Hartford Foundation awarded a three-year, $1.6M grant entitled, Moving and Scaling Home-Based Primary Care Phase II: Quality, Training and Advocacy. The project aims to improve care for the more than two million older adults who are the “invisible homebound” with functional impairments and frailty. This phase builds on the success of the initial grant phase, with the same organizations leading three complementary grants. While each organization has its own unique deliverables, they continue to work in close collaboration to achieve the overall goal ─ increasing access to high quality, home-based primary care that leads to improved outcomes for both patients and caregivers.
