Newswise — Through the American Medical Group Association (AMGA) Department of Quality Management and Research, the American Medical Group Foundation (AMGF), and Daiichi Sankyo, Inc., the association is providing grants for quality improvement initiatives focusing on the management of hypertension. Medical groups, IPAs, academic practices, and integrated delivery systems that have successfully incorporated the management of hypertension into their chronic care models are encouraged to submit a case study for a compendium of best practices.

This is an opportunity for provider organizations to showcase their hypertension management initiatives and be recognized as leaders in innovative, high-quality patient care. Those selected to be included in the compendium will receive $2500 educational grants to support their quality improvement efforts.

AMGA is most interested in population-wide efforts, but will also consider case studies for particular high-risk patient groups. A panel of judges, consisting of physicians and quality improvement experts, will be looking for programs that have successfully incorporated the management of hypertension into care processes from small, medium, and large practices and meet the following criteria:

"¢ Care provided in an outpatient setting"¢ Patient care is coordinated across the continuum of services"¢ Program results in significant improvements in patient health"¢ Program engages the entire care team (doctors including generalists and specialists, nurses, administrative staff, pharmacy, patients, etc.)"¢ Program allows for efficient, ongoing measurement and reporting"¢ Project design is practical and addresses program goals"¢ Intervention(s) are transferable to other practice settings

Featured compendium groups will be announced at the AMGA 2008 Annual Conference, March 6-8, 2008 in Orlando, Florida.
