SUMMIT DETAILSPersonal medication records (PMRs) are considered a critically important component of the continuity of care and medication reconciliation processes " and are a tool that could be used more effectively in improving the medication-use process.

The goals of the two-day Summit are to:"¢Reach consensus on a minimum set of data elements to be included in a PMR."¢Identify and discuss barriers and resolutions to the use of PMRs by patients, caregivers and health professionals."¢Develop a framework for further research and a national social marketing campaign aimed at enhancing patient, caregiver and health professional awareness of integrating the PMR in all aspects of the health care process.
