A large subgroup analysis of a randomized clinical trial showed neither a mortality risk nor benefit associated with the use of paclitaxel drug-coated devices (DCD) in the treatment of peripheral artery disease (PAD). Patients with left main coronary artery disease (LMCAD) typically have a poor prognosis due to the large amount of myocardium at risk. Revascularization with either percutaneous coronary intervention (PCI) or coronary artery bypass graft surgery (CABG) has been shown to prolong survival in patients with left main disease compared with medical therapy alone. Three-year data from the large-scale randomized ECXEL trial found no significant difference in the composite rate of death, stroke or myocardial infarction (MI) between the two treatments, with a reduction in 30-day major adverse events with PCI. A research letter was recently published in the Journal of the American Medical Association examining hospitalization, readmission, and mortality rates following pulmonary embolism (PE) in older US adults from 1999 through 2015. Researchers found that hospitalization rates for PE increased while length of stay, readmission rates, and short-term and one-year mortality rates declined. Investigators believe that these trends may be explained by advances in the diagnosis and management of PE, such as computed tomographic pulmonary angiography (CTPA), which is now the standard diagnostic test. Frequent use of CTPA may have contributed to identifying less severe PEs, hence the reduced mortality rates.
