CRANIAL BASE Correction of Progressive Hearing Loss in Superior Canal Dehiscence Syndrome.Eric P. Wilkinson, MD; Gene C. Liu, MD; Rick A. Friedman, MD, PhDSuperior semicircular canal dehiscence syndrome (SSCDS) results in vertigo and conductive hearing loss. Sound-induced vertigo is classically encountered. Dr. Wilkinson and colleagues report a patient with Tullio phenomenon, exercise-induced vertigo, and progressive mixed-hearing loss. A computed tomography (CT) demonstrated dehiscence of the superior semicircular canal. Middle fossa craniotomy allowed repair of the dehiscence. The patient experienced improvement in both bone and air conduction thresholds and immediate resolution of vertigo.

Devine, MSN, RN; Andrea Troxel, ScD; Soo K. Abboud, MD; Richard L. Doty, PhDIrradiation therapy impairs taste. Dr. Mirza and colleagues report a prospective evaluation of patients undergoing external beam radiation of the oral cavity. A validated assessment of taste was accomplished before and after therapy. Patients undergoing irradiation to other parts of the body served as controls. Head and neck cancer patients demonstrated decreased ability to identify bitter, sour and salt prior to therapy. Sour taste was highly susceptible to radiation, while sweet taste appeared the most robust. Two months after radiation some recovery was observed. Most taste had recovered at six months posttreatment, however bitter taste remained impaired. HEAD AND NECKClinical Features and Outcome of the Tall Cell Variant of Papillary Thyroid Carcinoma.Alex Kwok-Cheung Leung, MBBS; Sin-Ming Chow, MBBS, FRCR; Stephen C. K. Law, MBBS, FRCRTall cell variant (TCV) of papillary thyroid cancer is a relatively unusual but virulent form of papillary thyroid cancer. Patients so afflicted comprise 4%-12% of papillary thyroid cancer. Patients with TCV had a higher rate of extra thyroidal extension, tracheal invasion and carotid artery invasion. As a group, the patients are older and the mean tumor size is larger. Disease related mortality of TCV was 42.9% compared to only 7% in the entire cohort with papillary thyroid cancer. This study emphasizes the need to identify the patient population, and the need for improved therapeutic interventions. FREE Pretreatment Swallowing Exercises Improve Swallow Function After Chemoradiation.William R. Carroll, MD; Julie L. Locher, PhD; Cheri L. Canon, MD; Isaac A. Bohannon, MD; Nancy L. McColloch, CCC-SLP; J Scott Magnuson, MDDysphagia is a common sequela of chemoradiation therapy (CRT). Dr. Carroll and colleagues report a retrospective case control study of 18 patients scheduled for CRT. Patients were divided into two groups matched by cancer site, stage, and treatment. Pretreatment swallowing exercises were provided to one group while the control underwent only posttreatment swallowing exercises. Three months after completion of treatment, patients were assessed. This study provides evidence that pretreatment swallowing exercises do improve posttreatment function.
